BETWEEN PREDICATES AND PROTOCOLS: FUROR SANANDI, PRACTICAL FORMALISM, AND CASE-BY-CASE APPROACHES IN INSTITUTIONAL CLINICAL PRACTICE WITH TRANSGENDER PEOPLE AND TRANSVESTITES

This article examines ethical dilemmas in contemporary institutional clinical practice with transgender and transvestite people within a context marked by the proliferation of naming regimes – diagnoses, identity categories, and protocol devices – fundamental for guaranteeing rights, but problematic when they become exclusive clinical compasses. From the psychoanalytic principles, it argues that the subject of the unconscious cannot be reduced to the predicates that name it, such that no single predicate is sufficient to circumscribe being. The tension is analyzed between furor sanandi – excessive curative zeal – and practical formalism – understood as the reduction of clinical practice to the application of procedures, showing how, in institutional contexts permeated by social urgency and demands for efficiency, care can slip into tutelage. Finally, it proposes the defense of a case-by-case protocol as an ethical operator for directing treatment. It can be concluded that psychoanalysis maintains its function in institutional clinical practice when it resists both the logic of identity predication and the absolutization of protocols, preserving the subject's place within health practices.
